Prague Family Rural Health Clinic is owned and operated by Prague Community Hospial.  The clinic is staffed by Dr. Darryl Jackson, D.O., Dr. Scott Fowler, D.O., Bruce Carlton, PA-C,  and Mary Ann Ward, PA-C.

Dr. Darryl Jackson

Dr. Jackson began his full-time family practice in Prague, July 15, 2007.  He has been providing ER coverage part-time at the Prague Emergency Room since September 2006.

Dr. Jackson grew up in Kingston, OK, obtained a Bachelors Degree of Science in Biology from East Central University and  in 2004, received his Doctorate of Osteopathic Medicine from Oklahoma State University College of Osteopathic Medicine  in Tulsa.

He completed his internship at Tulsa Regional Medical Center in June 2005 and his Residency in June 2007 from the OSU Family Medical Residency program in Tulsa.  Dr. Jackson is certified in Advanced Cardiac Life Support, Pediatric Advanced Life Support, and Advanced Life Support in Obstetrics.

He received the Family Practice Award from Oklahoma State University Family Medicine Residency in May 2004. The Oklahoma Osteopathic Association (OOA) presented Darryl W. Jackson, DO with a “2009 Rookie of the Year Award” at the 109th Annual Convention. Dr. Jackson received the award for excelling in his field of family medicine during his first two years of practicing osteopathic medicine.

Mary Ann Ward, PA-C

Mary Ann provides care for emergency room patients Monday through Friday, 8:00 a.m. - 5:00 p.m. on a full time basis and also sees patients in the Prague Family Clinic.  She is practicing under the supervision of Dr. Darryl Jackson.

Mary Ann graduated from Paden High School in 2000.  She is the daughter of Charles and Darlene Cook of Paden, and is married to Shannon Ward, formerly of Prague.  She completed her Physician Assistant Program at the University of Oklahoma in November 2007 and received her certification in December.  She was a member of the honor society in her undergraduate program.  She is certified in Basic Life Support, Advanced Life Support and Pediatric Life Support.  She is a member of the OK Association of Physician Assistants, and the American Association of Physician Assistants.

Mary enjoys remodeling her home, reading, and spending time with her family which has recently grown to include a son Shannon "Gentry" born April 22, 2009.

Bruce Carlton, PA-C

Bruce Carlton, PA-C, began practicing at the Prague Family Clinic August 15, 2006.  Bruce was raised in Tishomingo and Edmond and graduated from the University of Oklahoma - College of Medicine Physician Assisitant Program.

He is a member of the Oklahoma Academy of Physician Assistants and is a nationally certified Physician Assistant.

Bruce practiced in Shawnee for 9 years, prior to coming to Prague, at the Shawnee Medical Center Clinic and Unity Health Center, North Campus, where he gained extensive clinical experience in urgent care, emergency medicine and internal medicine/family practice.

While in Shawnee, Bruce has been very involved in High School and Middle School sports where he assists students and coaches with sports medicine.  In his spare time Bruce enjoys riding his Harley and playing golf.

Dr Scott Fowler.

 Dr. Fowler attended Southwestern Oklahoma State University in Weatherford for his undergraduate studies. He has bachelors’ degrees in both Biology and Computer Science. While attending SWOSU, he held the position of nurse’s assistant at the local hospital where he confirmed his love of medicine.

He completed his Doctorate of Osteopathic Medicine from Oklahoma State University-College of Osteopathic Medicine in Tulsa. He recently completed his Family Medicine Residency at Oklahoma State Medical Center in Tulsa and is board certified in Family Medicine.
